This affects both the Origin version and the GOG version. The disc version, if fully patched, does not have this issue. I ask that EA please fully patch the Origin and GOG versions as well using the existing fixes provided by the patches for the disc version.

After reading your post, I attempted to restart Guadalcanal from the first Guadalcanal checkpoint to see if I could reacquire the Guadalcanal medals I originally earned them during a full playthrough , but my previous Guadalcanal medals were still saved which prevented this.
However, when I started from my last Makin Atoll save and finished the end of the Makin Atoll level again, it removed my Guadalcanal medals, thus allowing me to reacquire them.
I was able to reacquire all the Guadalcanal medals just a couple of hours prior to this post by starting from my most recent Makin Atoll save.
Note: With regard to Game Summary medals, if you win the game on a higher difficulty setting, you won't get the medals for the lower difficulty settings as well at least I didn't.
Whereas, if memory serves, when I completed Medal of Honor: Allied Assault on hard difficulty, I not only received the Distinguished Service Cross hard difficulty , but also the Silver Star normal difficulty and Bronze Star easy difficulty. Have a nice one. I was wondering, can you actually get all the medals that are awarded for beating the game on a certain difficulty? I ask because the way Pacific Assault reads what have you earned is a bit funky which I guess you found out yourself , I already applied the fix regarding the Guadalcanal rewards, I was simply thinking if I should even bother playing the game 4 separate times.
